The required ratio for children of the age three years old is one staff per ten children.
During an observation on 11/14/2025, Staff #1 left out of the cafeteria for approximately five minutes, leaving her class of 10 children, ages three-four years old, to be observed by two other teachers with a classroom of 20 children, ages three-four years of age. During the five minutes of absence, there were 30 children being supervised by two teachers inside of the cafeteria.

Plan of Correction: Staff #1 has been reminded of supervision requirements and the importance of maintaining ratio at all times. The coordinators will conduct random checks during meal times to ensure compliance.
