During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An initial subsidy health and safety inspection (SHSI) was initiated and completed on 09/30/2025. The inspection started at 10 am and concluded at 11:30 am. There were no children present and (1) caregiver. The inspector reviewed (6) caregiver/household members records on-site on 09/30/2025. Once children are enrolled, their files will be reviewed during the next inspection.

Information gathered during the inspection determined (6) areas of non-compliance with the applicable standards or law. These violations were documented on the violation notice issued to the program.

Standard 22.1-289.040-A
All adults (18 years or older) living in the home need to have a completed fingerprint check and a signed sworn statement on file.

There is currently no completed fingerprint check or signed sworn statement on file for Household Member #3.
Plan of Correction: HHM #3 just turn 18 several days ago. An appointment with Fieldprint of Virginia has been scheduled for this weekend. Once the results have been obtained, they will be shared and placed on file. A new sworn statement will be completed and placed on file.
Standard 8VAC20-790-260-K
Stairs shall not be accessible to children less than two years of age and children older than two years of age who are not developmentally ready to climb or descend stairs without supervision.

There is not a mounted safety gate that prevents access to the stairway that leads to the second floor of the home. The Vendor has requested to serve children from birth through 12 years of age.

There shall be an adult-sized toilet with a platform or steps and adapter seat available to a child being toilet trained.

There was not a step-stool or adapter seat available for the adult size toilet.

Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.
