Inspector Notes

An unannounced, on-site focused 60-day inspection was initiated and completed on 02/19/2026, as part of the conditional licensure period. The on-site inspection began at 1:15pm am and ended at 2:00 pm. The inspector reviewed compliance in the areas listed above.

There were 10 children present and 2 staff. The inspector reviewed 2 children?s records and 5 staff records on-site. This inspection included document review and tour of the facility.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 02/27/2026. A POC submitted after this date will not appear on the public website.

The center shall maintain and keep at the center a separate record for each child enrolled which shall contain all required information.

The records for Child #1 and Child #2 were missing the work phone number, and work address for the custodial parents; and addresses for the emergency contacts.
Plan of Correction: We will follow-up with families to enter any missing information.
Child #1 is out of the country and returns March 23, 2026.
