During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 3/11/2026, as a part of the conditional licensure period. The on-site inspection began at 10:35am and ended at 11:30am. The inspector reviewed compliance in the areas listed above. There were 3 children present and 1 caregivers; the point total was 6. The inspector reviewed 3 children?s records and 1 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 8VAC20-800-100-B
When physical examinations are completed prior to attendance, they shall have been conducted within twelve months prior to attendance for children two years of age through five years of age. The physical examination in the record for Child #1 was completed 1 year and 8 months prior to attendance. Child #1 was over 2 years old at first date of attendance. The physical examination in the record for Child #2 was completed 1 year and 10 months from the date of inspection. Although a first date of attendance was not provided, Child #2 began attending within the past 30 days. Child #2 is 2 years old.
Plan of Correction: Provider will request current physicals for Child 1 and 2.
Standard 8VAC20-800-50-A
Providers must keep a written record of children in attendance each day. The provider was open and providing care for children on March 10, 2026 but did not take written attendance.
Plan of Correction: Provider will document attendance daily.
Standard 8VAC20-800-60-B
An up to date record for each child enrolled shall be maintained by the provider and contain all required information. The record for Child #2 and #3 did not contain documentation of the first date of attendance. The record for Child #2 did not contain documentation of the phone number for the parents' place of employment. The record for Child #3 did not contain documentation of the address for one parent's place of employment.
Plan of Correction: Provider will document first dates of attendance in the record and request the additional information needed from the parents.
Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. An emergency evacuation was not practiced during the month of February 2026. Children were enrolled and in care during the month of February.
Plan of Correction: Provider will print the log and complete an emergency drill by the end of next week.
