During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 2/2/26. The on-site inspection began at 11 am and ended at 1:45 pm. The inspector reviewed compliance in the areas listed above. There were 49 children present and 10 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today which will be the close of business on 2/24/26. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-240-B
Staff shall complete orientation training in subsection C of this section prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

There was no documentation provided to show that staff A, B and C had obtained orientation training. This was overdue by 12 days for staff A and B and by 5 weeks for staff C.
Plan of Correction: Staff members signed orientation document to reflect their initial orientation
training upon hire.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean condition.

After changing a toddler, the staff and the child touched two door handles in the diapering area before washing their hands. The door handles were not cleaned afterward.
Plan of Correction: Staff member was immediately directed to use the sink designated for diaper changing. Retrained teachers in classroom.

The center's procedures for administering medication shall include duration of the parents authorization for medication, provide that it shall expire or be renewed after 10 work days. Long-term prescription drug use and over the counter medication may be allowed with written authorization from the child's physician and parent.

The center's name was not identified on the authorization form signed by the parent/physician for two long term medications for child A.
Plan of Correction: Parent provided signature on Medication Form to reflect the school's name as authorized to administer medication, prior to the conclusion of the visit.

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided that the center has obtained written authorization from a parent/guardian.

There was no parental authorization for a medication for child A.
