When staff are supervising children, they shall always ensure their care, protection, and guidance.
On 1/20/26, staff A fed infant A ( 4 months of age) approximately 2.5 ounces of breastmilk from another infant's bottle.

Plan of Correction: School self-reported (left message) on the evening 1/20/26. Inspector followed up with school the next morning 1/21/26.Conducted a detailed investigation of incident;
- School informed both parents involved, immediately;
- Self-reported incident to Company Franchise and VDOE licensing;
- Support teacher completed a redo of all Health and Safety training;
- All infant and support staff were retrained on safety and feeding protocols;
- Adopted a bottle-verification step (initialing bottles);
- Staff meeting was held to reinforce expectations and safety procedures
