The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

Standard 8VAC20-780-270-A
Areas and equipment inside and outside the center shall be maintained in a clean, safe, and operable condition.
1. In the school age room, there was an electrical outlet cover hanging down attached by an electrical cord. Also, the counter with the sink and the other countertop nearby have both detached from the backsplash created a gap that could be a pinching hazard.
2. The wood fencing surrounding the baseball diamond on the outdoor playground had chipped and peeling paint on all the slats.
3. The fence between the toddler and the older children's outdoor playground had six slats that were attached only at the bottom and not at the top causing them to lean different directions posing an impalement hazard.
4. On the older children's playground there were four metal stakes used for the resilient surfacing barrier that were protruding up approximately one to three inches.
5. The wood trim going down the length of the double slides on the water park was not properly attached causing gaps in certain areas and poses a snag and entanglement hazard. The wood trim on the step area at the top of these double slides also had several protruding nails.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards, and shall be under equipment with moving parts of climbing apparatus to create a fall zone free of hazardous obstacles.
1. The mulch at the base of the blue double slide and blue single slide, both measuring 33 inches in height, had only one inch of resilient surfacing when six inches of resilient surfacing is required.
2. The mulch at the base of the green curved slide, measuring 58 inches in height, had only one inch of resilient surfacing when six inches of resilient surfacing is required.
3. In the waterpark area, the water slides, measuring 42 inches in height, had wood fencing ten inches from the slide, however, six feet of fall zone is required.
4. In the waterpark area, poured in place concrete is utilized as resilient surfacing, however, the poured in place is cracked in several areas within the six foot fall zone surrounding the slides and equipment making the surface no longer resilient.
