Inspector Notes
An unannounced, on-site initial inspection was initiated and completed on 1/21/2026, as a part the initial licensure period. The on-site inspection began at 10:00 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were no children in care at the initial inspection. The inspector completed a document review including model forms for Injury logs, policies and procedures, emergency response planning emergency drill logs, medication administration logs, authorization forms, tour of the facility, interviews, observations , and measurements.
