During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site focused inspection was initiated on 03/13/2026 and completed on 03/13/2026, as a part of the conditional licensure period. The on-site inspection began at 9:28am and ended at 11:18am. The inspector reviewed compliance in the areas listed above. There were 44 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined noncompliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-500-A
Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

In classroom #3, three separate children used the toilet without washing their hands after, then returned to the table for an activity.

Staff shall wash their hands with soap and running water before and after a diaper change.

One staff in the infant room did not wash hands before or after a diaper change for one child.
Plan of Correction: During our next staff meeting on March 18, 2026, all staff will be retrained on proper handwashing.
