Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Documentation of viewing proof of the child's identity and age.

Plan of Correction: The citation was corrected the same day, the documentation was obtained and verified, and we will ensure ongoing compliance by reviewing all student files upon enrollment and conducting monthly audits to confirm that proof of identity and age is properly documented for every student.
