The program is required to submit results of potable water testing to VDH. The program did not have evidence of submitting test results to VDH.
Plan of Correction: Center management will complete the required water testing and submit the required documentation to VDH office of drinking water once the testing has been completed.
Standard 8VAC20-780-270-A
Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. There are wooden cubbies that are approximately 4 feet tall that are not secured to the wall and pose a toppling hazard. There is baseboard flashing missing in one bathroom.
Plan of Correction: Photos showing that all cubbies have been secured to the wall and the baseboard flashing has been installed.

Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards. There are three pieces of playground equipment that do not have any resilient surfacing under or around them.
Plan of Correction: Caution tape has been placed on and around the three spring bouncers so that they cannot be used until resilient surface has been installed.
