Providers must maintain a record for each caregiver which contains all required information. The record for Caregiver #2 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Providers must maintain a first aid kit that contains all required items. The first aid kit was missing an additional triangular bandage.
Plan of Correction: Provider will purchase an additional triangular bandage today.
