Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment.

The sworn statement for Staff #4, employed for approximately two weeks, was completed after the start of employment.
Plan of Correction: The sworn statement that was not in staff folder, we stapled to keep paper from falling out. This was corrected immediately.
Standard 22.1-289.035-B-2
The center must obtain a completed national criminal background check prior to the employee's first day of employment.

Staff #1, Staff #2, Staff #3, and Staff #4, all employed for approximately two weeks did not have a completed national criminal background check. Staff #5, employed for approximately one day, did not have a completed national criminal background check.
Plan of Correction: All staff had prior backgrounds checks with prior employment. Submitted portability, was not aware we had to wait pending. Going forward, no staff will be hired without approval letter from portability.

The center requested central registry searches for Staff #1, Staff #2, Staff #3 and Staff #4, two days after the start of employment.
Plan of Correction: All staff had prior backgrounds checks with prior employment. Submitted portability, was not aware we had to wait pending. Going forward, no staff will be hired without approval letter from portability.
