During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An announced, on-site initial inspection was initiated and completed on 3/24/2026, as a part of the licensure period. The on-site inspection began at 9:00am and ended at 11:00am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 2 caregivers; the point total was 17. The inspector reviewed 5 children?s records, 2 caregivers, and 1 household member records on-site. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.

The immunization record for Child #1, Child #2, Child #3, and Child #5 were not signed.
Plan of Correction: Provider will reach out to families to provide a signed or stamped immunization record by the doctor.
Standard 8VAC20-800-330-A
A landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. An operable landline telephone is one that does not require electricity to operate.

The telephone observed was cordless and required electricity to operate.

Standard 8VAC20-800-60-A
The provider shall maintain an up-to-date record at the family day home for each enrolled child.

The record for Child # 1, Child #2, Child #3, Child #4, and Child #5 was missing parent's signed acknowledgement on the provider's policy of administering medication, whether or not there is liability insurance in force on the family day home operation, and the results of the physical examinations.
Plan of Correction: I will provide the required information to parents and receive the physicals from parents.
Standard 8VAC20-800-690-D
The diapering surface shall be cleaned and sanitized after each use.

The diapering area did not have a soap-and-water cleaning solution or an EPA-approved sanitizing solution available.
Plan of Correction: The provider will use an EPA approved sanitizing solution and use a soap and water solution to clean the diapering surface.
Standard 8VAC20-800-690-E
Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

The trashcan was not foot-operated and could not be opened without touching its exterior surface.
Plan of Correction: I will buy a foot pedal trashcan.

Bottles shall be labeled with the child's full name and the date, if more than one infant is in care.

Five bottles were observed that did not include the child's full name and the date.
Plan of Correction: I will label the bottles daily to include full name and the date.
