Playground equipment with moving parts or climbing apparatus must have appropriate resilient surfacing in the fall zones under and around the equipment. There was approximately 4" depth of mulch on either side of the red plastic slide when there should be at least 6" depth.
Plan of Correction: The slide was removed immediately during inspection.
