Inspector Notes

An unannounced, on-site monitoring inspection was initiated on December 16, 2024 and completed on December 16, 2024. The on-site inspection began at 10:50 a.m. and ended at 12:15 p.m. The inspector reviewed compliance in the areas listed above. There were 31 children present and nine staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews and observation.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on December 26, 2024. A POC submitted after this date will not appear on the public website.

At least every two years, staff members shall obtain and submit results of a follow-up tuberculosis (TB) screening.
Staff #1?s follow up TB screening was six weeks past due.
Plan of Correction: I wrote down the wrong year. I will make sure to recheck my work to make sure the dates are correct.
