Inspector Notes

A complaint inspection was conducted on 8/31/2022 from 11:15 a.m. to 2:15 p.m. regarding a complaint received on 8/19/2022 related to staff qualifications, supervision, ratios and terms of the license. There were 45 children present, ranging in ages from 5 months to 5 years old, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, staffing and supervision, A total of 2 child records and 1 staff record was reviewed.

Evidence gathered during the inspection supported two of the allegations and the complaint was determined to be valid. Two violations were issued.

Based on observation, interviews and review of records, the center failed to maintain the required ratios.
Evidence:
1. One teacher and eight children were observed in Classroom #1.
2. Based on review of records, the Child #2 was 1 year and 10 months old at the time of inspection.
3. Staff #2 and Staff #4 confirmed that child #2 was younger than 2 years old and that the classroom was not within the required ratio of 1:5.
Plan of Correction: Unaware of 2 additional assessments per new regulations. Will attach assessments to agreement letter.

Based on interview and review of records, the center did not maintain compliance with the terms of the issued license.
Evidence:
1. The center is licensed for ages 0 years, 1 month through 6 years, 11 months as stated on the license.
2. According to the record for Child #1, the child?s birthday is 07/22/2014 and the child was enrolled on 06/19/2017.
3. Attendance sheets showed Child #1?s last day of attendance was 07/19/2022. Staff #2 and Staff #3 confirmed this date was accurate.
3. Staff #3 confirmed that child #1 was 7 years, 11 months old on the last day of attendance.
Plan of Correction: Unenrolled child and will request age limit to 12 years. Learned children of staff may be in classrooms unenrolled - did not know that.
