Inspector Notes

An unannounced renewal inspection was conducted on 10/26/2023 from 10:30 a.m. to 12:30 p.m. There were 37 children present, ranging in ages from seven months to four yeas old with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the center failed to meet the required staff: children ratio of 1:5.
Evidence:
1. On the back playground, there was two staff with 15 children.
2. Staff #4 confirmed the youngest child in the group was Child #1.
3. Records and Staff #4 confirmed the age of the youngest child in the group was 18 months old.
Plan of Correction: 1 classroom outside at a time so if a child runs over to talk to another teacher who has her class playing in a designated play area (sandbox) it will not be construed as comingling 2 age groups.

Based on observation and interview, the center failed to meet the required staff: children ratio of 1:10.
Evidence:
1. In the three-year-old classroom, Staff #6 was observed supervising 11 children.
2. Staff #6 confirmed the age of the youngest child in the group was three years old.

Based on observation and interview, the center failed to ensure that when food is brought from home, it is clearly dated and labeled in a way that identifies the owner.
Evidence:
1. In the preschool classroom, there were four lunchboxes that were not labeled with a date, 15 cups that were not labeled with a date, four lunchboxes that were not labeled with a date or a name and four cups that were not labeled with a date or name.
2. In the toddler classroom, was one lunchbox that was not labeled with a date, three cups that were not labeled with a date and one lunchbox that was not labeled with a date or a name.
Plan of Correction: Will be more mindful about checking names/dates
