Inspector Notes

A complaint inspection was conducted on January 11, 2023 from 1:30 P.M.-2:50 P.M. A complaint was received in the Office of Child Care Health and Safety (Valley) on January 10, 2023 regarding physical abuse. The inspector reviewed compliance in the areas of administration, staffing and supervision, programming, and special care and emergencies. A total of one child record was reviewed, four staff interviewed, attendance records, injury reports and video footage were reviewed.

The information gathered during the inspection did not determine preponderance of evidence. Therefore the complaint is not valid.

There was one violation not related to the complaint that was not in compliance with applicable standards or laws, and violations were documented on the violation notice issued to the program.

Based on review of injury reports, the center failed to ensure that all information was included on the report.

Evidence:
1. An injury report dated 01/09/2023 was missing the date and time the parent was notified of and a second signature from a parent or another staff member.
2. Administration verified that the report was missing all required information.
Plan of Correction: Staff will be retrained on the requirements when completing an injury report. The parent was notified at pick-up, but was in a rush and did not sign the report.
