Inspector Notes

A mandated monitoring inspection was conducted on March 8, 2023 from 11:00 P.M.-2:00 P.M. There were 32 children present, ranging in ages from two months to five years of age, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on review of staff files, the facility failed to ensure that a tuberculosis screening had been completed within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #2's date of employment was 02/24/2023. The tuberculosis screening on file was dated 03/18/2021.
2. Staff #1 verified the date of employment for Staff #2, and the date on the tuberculosis screening.
Plan of Correction: The staff member has been scheduled to get an updated tuberculosis screening. In the future administration will ensure that TB tuberculosis screening for new employees are obtained in the correct timeframe.

Based on a review of the emergency drill log and interview, it was determined that the facility did not ensure that a lockdown was practiced at least annually.

Evidence:
1. There was no documentation that a lock down drill had been completed in 2022.
2. Staff #2 verified that a lockdown drill had not been completed in 2022.
Plan of Correction: A lockdown drill we be scheduled and completed this week. Lock down drill will be added as a line item for our documentation to add a visual reminder to complete annually.
