During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on June 24, 2025. The on-site inspection began at 12:15 p.m. and ended at 2:25 p.m. The inspector reviewed compliance in the areas listed above. There were 48 children present and 12 staff. The inspector reviewed four children?s records and two new staff records on-site June 24, 2025. This inspection included document review, tour of the facility, and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #1, employed for two months, did not have documentation of requesting a central registry check , criminal history and sex offender for one state. Staff #2, employed for over one week, did not have documentation of requesting a central registry check , criminal history and sex offender for one state.
Plan of Correction: The director immediately located the state for which the out of state checks need to be requested and prepared the documents. The sex offender checks were completed while the inspector was still on site. The interstate background check list was bookmarked so no oversight occurs in the future.
