Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on-site September 25, 2023. There were 37 children present with 7 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Repeat Violation
Based on a review of records and interview, the vendor failed to obtain current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care.
Evidence: The record of staff #4 did not contain documentation of current certification in CPR. The most recent CPR certification for staff #4 is dated 09/02/21 and that was an online only course. Staff #4 confirmed there wasn't a current CPR certification.
Plan of Correction: Staff #4 will complete 1st Aid/CPR certification & maintain such certification while employed at center.

Evidence: The record of staff #4 did not contain documentation of current certification in first aid. The most recent first aid certification for staff #4 is dated 09/02/21. Staff #4 confirmed there wasn't a current CPR certification.
