Inspector Notes
An unannounced complaint inspection was conducted using a remote protocol from June 20, 2023 to September 25, 2023. The department received a complaint regarding an injury to a child. The inspector reviewed compliance in the areas of staffing and supervision, interviews were conducted, and records were reviewed.
The evidence gathered during the inspection determined the complaint was not valid and there were no violations with applicable standards or law. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
1Evidence: In an interview with staff #1 it was stated that on 06/08/2023, child #1 was taken to the hospital for a suspected injury. Staff #1 stated the center was made aware that child #1 received outside medical care on 06/09/2023. This incident was not reported by the facility to the office of child care health and safety.
