Inspector Notes

A subsidy health and safety (SHSI) inspection was conducted on-site September 11, 2024 from 10:02am until 11:15am. There were 35 children present with 9 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 23, 2024. A POC submitted after this date will not appear on the public website.

Based on a review of staff records and interview, the vendor failed to ensure that each staff complete the Virginia Annual Health and Safety Update Training annually as required.
Evidence: The record of staff #5 contained documentation of the annual update training dated 6/12/23. There was no documentation of the 2024 update training. Staff #7 confirmed the 2024 was not completed.
