Inspector Notes

A monitoring inspection was conducted on December 20, 2022. There were forty-seven children present, ranging in ages from three months to five years, with eleven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and seven staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Based on review of seven staff files, program failed to ensure that a sex offender registry check and search of the child abuse and neglect registry from any state in which an individual has resided in the preceding five years was completed for two staff.

Evidence:
1. The first day of employment for Staff #2 was 10/10/2022. There were no out of state sex offender check in the file nor out of state child abuse and neglect search results.
2 The first day of employment for Staff #5 was 3/7/2022. There were no out of state child abuse and neglect search results in the file.
3. The administrator checked the files and confirmed these items were not in the file.

Based on review of seven staff files, the program failed to ensure that the results of a central registry check are in the file within 30 days of employment.

Evidence:
