Inspector Notes

A monitoring inspection was initiated on 11/15/2023 and concluded on 11/15/2023 from 10:00 AM to 1:00 PM. There were 49 children present, ranging in ages from infants to five, with 13 staff and three administrators. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of five children?s records and the five staff?s records were reviewed.
Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard 8VAC20-780-40-M
Based on interview, the center failed to obtain an allergy care plan for a child with documented food allergies.

Evidence:
An inspection of the allergy list in each classroom was completed. The administrator was unable to provide an allergy care plan.
Plan of Correction: Child 7's parents will be advised to provide an allergy care plan within a week or the child will not be allowed to return until the document is provided. In the future no child will be allowed to start at the center until the allergy care plan is obtained.
Standard 8VAC20-780-530-A
Based on observation and interview, the center failed to ensure at least one staff in each classroom is certified in CPR and first aid.

Evidence:

1. Staff 6 was observed alone with the Stars class. The administration stated staff 6 was the only teacher with the Stars class.
2. The administrator was unable to provide an updated CPR and first aid certificate for staff 6.
Plan of Correction: A class will be scheduled as soon as possible. In the future the administrator will be required to keep track of all staff first aid and CPR certificates to ensure they do not expire.
Standard 8VAC20-780-70
Based on record review and interview, the center failed to keep a staff record that processed documentation to demonstrate the education, certification, and experience required by the job position.

Evidence:

1. The record for staff 2, start date documented as 10/6/22, was reviewed. The resume in the staff record did not indicate education or certification to qualify for lead and did not indicate programmatic experience.
2. The director stated that staff 2 is the lead teacher in the classroom and did work as a lead in a license center prior to coming to work at this center.
Plan of Correction: An updated staff resume was obtained during the inspection. In the future the administration will ensure the staff records have documentation to indicate how the staff member qualifies for the position.
