During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced on-site inspection was initiated on January 16, 2025 and completed on January 16, 2025 in response to a complaint received by the licensing office on December 19, 2024 relating to allegations of supervision of children. The inspector reviewed two staff records and center policies and procedures. This inspection included document review, tour of the facility, and interviews conducted with a local agency on January 14, 2025 and January 21, 2025. The preponderance of evidence gathered during the investigation supports the allegations; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on January 30,2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff. On December 18, 2024, a parent reported to the center that a child was on the playground alone. Staff confirmed that child #1 (age two) was then found crying and alone on the playground. Per staff #3 the child was on the playground alone for about four minutes.
Plan of Correction: Reviewing our name to face policies and headcount procedures (done at every threshold and transitions).
Staff must communicate prior to breaks and leaving for the day any issues, headcounts, and name of children in care. The center also updated the missing child policy.

REPEAT VIOLATION The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.
On December 18.2024, two staff did not follow the center's procedures for identifying where attending children are at all times which resulted in a two-year-old child being left alone on the playground for approximately four minutes before a parent intervened.. The playground is located near a major university, surrounded by apartments and regular foot traffic.
Plan of Correction: The day after the incident the center reviewed the name to face policies and accountability procedures with staff. Each staff member has signed an updated name to face policy ensuring that they understand our procedures. Additional training in the Tadpoles app will be provided to all staff. We all created a new guideline that all staff must stay with the group during transitional times for an additional five minutes after the transition from one place to another.
