During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated on August 11, 2025 and completed on August 11, 2025, as a part of probation and provisional licensure period. The Department issued a Special Order on April 15, 2025 to place license on probation and require licensee to contact parents, guardians or other responsible persons as a result of high-risk violations. The on-site inspection began at 10:20 a.m. and ended at 1:40 p.m. The inspector reviewed compliance in the areas listed above. There were 45 children present and 15 staff. The inspector reviewed five children?s records and five staff records on-site. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. In the restroom used by children in the Stars room, hazardous substances such as Lysol Cleaner and Windex were found on a shelf above the toilet.
Plan of Correction: All hazardous substance were moved to a locked location and staff were reminded to keep hazardous substances in a locked location.

Over-the-counter skin products shall not be kept or used beyond the expiration date of the product. Four diaper ointments were expired.
Plan of Correction: The diaper ointment was removed from the room and will be returned to the parents. Staff were reminded to check diaper ointments monthly for expirations.
