Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence that contains date & time of injury, name of injured child, type and circumstance of the injury, staff present and treatment, date and time when parents were notified, any future action to prevent recurrence of the injury, staff and parent signatures or two staff signatures and documentation on how parent was notified.
Three injury reports reviewed for child #1 did not contain staff present and treatment, date and time when parents were notified, any future action to prevent recurrence of the injury, and staff and parent signatures or two staff signatures.
Plan of Correction: A new written form that contains all the required components will be used from now on.
Standard 8VAC20-780-80-B-2
The center shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention. An injury that took place three weeks ago and resulted in a child going to the emergency room was never reported to department. The center was aware the child went to the emergency room.
Plan of Correction: The administrator is now aware of this requirement and will ensure to complete this report if any future incidents that lead to professional medical attention takes place.
