Inspector Notes

An unannounced, on-site monitoring and focused inspection was initiated on February 26, 2026 and completed on February 26, 2026, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as a result of the last inspection related to administration and programing. The on-site inspection began at 11:20 a.m. and ended at 2:10 p.m. The inspector reviewed compliance in the areas listed above. There were 61 children present and 15 staff. The inspector reviewed six children?s records and six staff records on-site. This inspection included a tour of the facility, record review, observations, and interviews. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
