Inspector Notes

A complaint investigation was initiated on 01/21/2022 and concluded on 05/03/2022 in response to a complaint that was received by the department on 01/10/2022 regarding staff qualifications and background checks. The inspector conducted an unannounced inspection on-site on 01/21/22 from approximately 10:10am to 11:35am. Interviews with staff were completed, observations were made at the center, and documentation was reviewed. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint was determined to be not valid. There are no violations resulting from this inspection.
