Inspector Notes

A monitoring inspection was initiated in response to a self-reported incident regarding injury reporting that occurred at the center on 06/02/2022. The Department was notified of the incident on 06/06/2022. The investigation was initiated on 06/06/2022 and completed on 08/29/2022. On 06/15/2022, the inspector conducted an unannounced inspection from3:40pm to approximately 4:30pm. Interviews were completed, observations were made in the center, and documentation was reviewed. Additional interviews were conducted off-site and additional information was submitted to the inspector electronically. The preponderance of the evidence gathered during the investigation did not support violations of the standards. There are no violations resulting from this inspection.
