Inspector Notes

An unannounced monitoring inspection was conducted on July 3, 2023 from approximately 11:30 am to 1:00 pm. There were 19 children in attendance and a total of 5 staff present. 4 staff records and 4 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playgrounds. The children were observed getting ready for lunch, and watching an educational program while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 07/21/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on records review, the center failed to ensure that employees shall undergo a background check prior to employment and every five years thereafter. .
Evidence:
The record for staff #3, date of employment 06/15/1988, contained criminal background check results dated 06/21/18. A criminal record check should have been conducted by 06/21/2023.

Standard 8VAC20-780-60-A-8
Based on records review, the center failed to obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: The record for child #1, date of enrollment 02/28/19, indicated an allergy to milk. There was not a written care plan from a physician.
