Inspector Notes
This inspection was amended on 03/25/2025 to correct the inspection dates. An unannounced inspection was initiated and completed on 2/28/25 in response to a complaint received by the licensing office on 2/19/25 relating to allegations of building maintenance, ratios, records, and behavior management. The inspector reviewed 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
