During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced monitoring inspection was initiated on 05/06/2025 and completed on 05/08/2025 in response to a self-report received by the licensing office on 04/30/2025 relating to supervision of children. The inspector reviewed 1 child record and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 05/23/2025. A POC submitted after this date will not appear on the public website.
Violations
1Review of video footage and interviews revealed that at approximately 4:30 pm on 04/29/2025, child #1, age 3 years, left the playground and ran through the parking lot. Staff #1 and staff #2 were unaware that child #1 left the playground. Several cars were pulling into and out of the parking lot at the time. The center is located on a high traffic road.
2. Pickup Procedure Changes: Outdoor playtime has been rescheduled so that all children are indoors by 5:00 PM, which coincides with peak pickup times. This minimizes outdoor activity during high traffic periods and ensures safer supervision during transitions.
3. Staff training and Supervision Protocols: All staff has been retrained on playground supervision policies, with a strong emphasis on sight and sound monitoring, proper positioning, and designated safety zones. Staff are now assigned to specific zones within the playground to ensure consistent, overlapping coverage of all children during outdoor play. A playground supervision checklist has been implemented and is reviewed daily to ensure compliance.
4. Supervisory staff will conduct unannounced spot-checks during outdoor playtime to ensure proper supervision and adherence to the new procedures. Monthly safety drills and quarterly staff refreshers on supervision protocols will be added to the professional development schedule.
