During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes
An unannounced, on-site focused inspection was initiated and completed on 10/10/2025 , as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to operational responsibilities, children's records, supervision, and out of state background checks. The on-site inspection began at 12:01 pm and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above. There were 18 children present and 4 staff. The inspector reviewed 2 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
