During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on November 14, 2024 and completed on November 22, 2024. The on-site inspection began at 9:30am and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 149 children present and 27 staff. The inspector reviewed 10 children?s records and 10 staff records on-site. Board Officers? information were viewed electronically on November 22, 2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on December 6, 2024. A POC submitted after this date will not appear on the public website.

The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The record for Child #10 contained documentation of immunizations obtained 11 days after the first day of attending the center.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire. Staff #2, who is working with children, has not yet completed this course which is overdue by five months.

Hazardous substances must be in a locked place that prevents access by children. In a two-year-old classroom, containers of cleaning chemicals were in an unlocked cabinet, out of reach of children.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. An uncovered outlet was located in the bathroom used by the preschool children.

Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #5 and Child #6 has a diagnosed food allergy documented in the record. The records did not contain a written care plan. Child #5 and Child #6 has attended the center for five months.
