The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. A temporary access door was cut into the bathroom wall located in Classroom A. The temporary access door was a metal door that did not lock. The metal edges of the temporary door and the plumbing located in the bathroom wall were accessible. Children were observed using the bathroom during the inspection.
Plan of Correction: A temporary fix will be made to make the door inaccessible. A work order was submitted during the inspection to create a safe and long term solution.
