Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 10/27/2021. There were 35 children present with 8 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the vendor did not ensure to obtain the results of a negative tuberculosis screening for one staff at the time of employment and prior to coming into contact with children.

Evidence:
1. The record of staff #1 (DOH: 9/17/2021) contained a tuberculosis screening dated 09/27/2021.
2. Administration acknowledged that the tuberculosis screening was completed after hire.
Plan of Correction: Management will be sure that all new hires have TB test done prior to coming into contact with children.
