Inspector Notes
A renewal inspection was conducted Today, January 23, 2024. The licensing inspector was on site from approximately 9:55 am to 1:20 pm. The center director was on site and assisted with the inspection. There were 27 children present with five staff supervising. Children in care were observed reviewing the letter N, counting numbers, building blocks and dancing to music. Staff was observed singing and playing with an instrument for the infants. The inspector reviewed compliance in the areas of administration, physical plant, staffing, supervision, programming, injury reporting, medication, special care
and emergencies and nutrition. Medication is administered and was reviewed during today?s inspection. A total of five child records and three six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Evidence:
1. The record of Agent #4 contained documentation of a national fingerprint background check dated 04/03/2018, exceeding five years.
2. The director stated the applicant had not corrected missing information from the request to renew the national fingerprint check and therefor the national fingerprint had not been completed.

The record of staff #1, employed 08/10/2023, contained a sworn statement dated 08/11/2023 exceeding the 1st day of employment.
