Standard 8VAC20-780-240-D
Prior to working alone with children and within seven days of the first day of employment, staff shall be provided in writing with the center's procedures required by this standard.

1. Management reported that the center does not have written procedures for supervising a child who may arrive after scheduled classes or activities including field trips have begun and procedures for identifying where attending children are at all times, including procedures to ensure that all children are accounted for before leaving a field trip site and upon return to the center.

2. Management reported that these procedures are reviewed verbally with staff during orientation.
Plan of Correction: Management did not report that the Center does not have a written procedure for supervising a child. Management reported that the supervision was written throughout the employee and parents handbooks and reviewed and signed off on at each and every employee orientation. We did not have a dedicated policy in writing that was specifically a Supervision policy. We have compiled a separate and detailed Supervision Policy for both our Employee and Parents Handbooks. We will continue to include this in our employee orientation and have each employee sign and date. Also as part of out POC, Advent scheduled a mandatory staff meeting where we hired Child Savers to train us in a workshop titled "Active Safety and Supervision in the Child Care Setting". Every staff member was present and received completed certificates. We will continue to review this information with every employee at orientation but Advent will have this in writing in our revised employee handbook which each will receive. This will be documented by signing and dating that each received prior to working alone with children and within seven days of the 1st day of employment. With all of our present employees, they have had a Supervision Policy Orientation at which time, they have received this information in writing with out revised employee handbook. They are signing and dating that they have received.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 12/11/25, video footage showed the following:
1. Child #2 (3 years old) exited room #1 through the adjoining bathroom and entered room #2. Room #2 did not have staff or children present.
2. Staff #2 was not aware that child #2 left room #1. Staff #2 exited the room with other children, leaving child #2 in room #2 alone and unsupervised.
3. After more than 5 minutes, child #2 went back to room #1 and exited the room, went down the steps to the first floor, through the building to the door of room #3, pulled on room #3?s door and a staff member opened the door.

On 12/17/25, video footage showed the following:
1. Child #1 (2 years old) was on the playground unsupervised for one minute.
2. Staff were unaware that child #1 had been left on the playground until they heard the child knock on the door.
Plan of Correction: Per out revised Supervision Policy found in out Employee and Parents Handbooks, this is the first line of our Policy that the purpose of the policy is to ensure the safety, well-being, and development of all children providing constant, active, and appropriate supervision at all times while in our care. We have created a Pre-school Coordinator only role. Our Coordinator is the main person in charge of out total count at all times. Our teachers are responsible for letting the Coordinator know by walky talky whenever a child is dropped off or picked up and responsible for sure that the classroom and the total tally are in sync throughout the day and if not to find out why. All transitions, the children will be counted and matched to the class list. The class lists are to be with the teachers at all times. The counts will happen when we line up, when we are moving and when we get to where we are going. They will be counted by all staff that is present with them whether in the classroom or playground or bathroom, etc. Advent has also purchased ropes with handles for our 2 year old classes to hold onto while they are transitioning. We have always done a playground sweep after leaving our playgrounds with a designated person, but, per our plan of correction we will have a designated sweep person for each playground.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 12/11/25, video footage showed the following:
1. Child #2 (3 years old) playing in room #2 alone and unsupervised.
2. Child #2 was not within actual sight and sound supervision of a staff member for six minutes.

On 12/17/25, video footage showed the following:
1. Child #1 (2 years old) was left unsupervised on the playground for one minute.
2. Child #1 knocked on the door of room #1 and a staff member opened the door.
Plan of Correction: We have pulled one of our Lead Teachers and made her out Pre-School Coordinator only, she was doing both jobs. Now she is responsible to oversee the many daily transitions that go on at our center especially on different days when an event is happening at the center like what happened on 12/11/25 (Christmas Program). She has a complete tally of the number of all the children in each classroom. She will be on the walky talky getting counts after every transition during the day. She will make sure that all classes have their class list with them at all times. All teachers will be reporting throughout the day whenever a child gets picked up or dropped doff and report their count at that time which the Coordinator will then adjust on her Master tally list. The coordinator will also do a classroom sweep of all classrooms used during an event at the center following the event.
Presently, we do a playground sweep after the last class goes into the building. The playground sweep person inspects both playground, calls out all clear and says to check your counts. We have a record of this daily. Moving forward, we will have a playground sweep person for each playground especially since on the 2 year old playground exits a little earlier.
