During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced monitoring inspection was initiated on March 17, 2026 and completed on March 19, 2026 in response to a self-report received by the licensing office on March 16, 2026 relating to staffing and supervision. The Department issued a Notice of Intent to place licensee on probation and require licensee to contact parents, guardians, or other responsible persons on March 5, 2026 as a result of high-risk violations. The inspector reviewed one child?s record and two staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to handwashing procedures and immunizations for children and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on March 26, 2026. A POC submitted after this date will not appear on the public website.
Violations
5The immunization record for child #1, enrolled for 13 days, was not signed or dated.
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On March 13, 2026, video footage showed the following:
1. Staff #2 was supervising three children in room #1. One child exited the door to the playground, and another child followed. Staff #2 ran after both children, leaving child #1 (two years old) unsupervised in room #1.
2. Two adult family members of another child enter room #1.
3. A second staff (Staff #1), entered room #1, greeted child #1 and exited the door to the playground leaving child #1 with another child?s family members. Staff #1 assumed the family members were there to pick up child #1.
4. A third staff entered and exited room #1 without noticing child #1.
5. After approximately two minutes, the family members notified staff on the playground that child #1 was in room #1 unsupervised.
Children under 10 years of age always shall be within actual sight and sound supervision of staff.
On March 13, 2026, video footage showed the following:
Staff #2 exited room #1 with two children leaving child #1 (2 years old) unsupervised in room #1 for two minutes.
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures required by the standards.
1. On March 13, 2026, video footage showed child #1 (two years old) in room #1 unsupervised for approximately two minutes.
2. The center's supervision policy states: "The purpose of this policy is to ensure the safety, well-being, and development of all children by providing constant, active, and appropriate supervision at all times while in our care.?
1. Child #1?s hands were not washed after diapering.
2. Staff #2 did not wash their hands after diapering.
