The center is required to have employees undergo a background check every five years. The record for Staff #1 contained a Sworn Statement dated 10/12/2020. The records for Staff #2 and Staff #3 contained a Sworn Statement dated 12/03/2025. The updated Sworn Statements were obtained one month and 21 days after the required timeframe.
Plan of Correction: I discovered the Sworn Statements were overdue when auditing the staff records. As soon as this was discovered, I gave the Staff the forms to update.
