During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on October 15, 2024 and completed on October 15, 2024. The on-site inspection began at 10:05 AM and ended at 1:10 PM. The inspector reviewed compliance in the areas listed above. There were 37 children present and 8 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included, document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 20, 2024. A POC submitted after this date will not appear on the public website.
Violations
5The record for staff #5 had a TB screening seven days past the date of the last TB screening.
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.
1. In the infant room, the backdoor double-pane window had a scattered windowpane.
2. On the playground, a total of eight fence boards were broken, pulled away from the fence base or missing.
3. In the toddler room, the wall was cracking and damp to the touch.
4. In the Pre-K room, the molding was missing exposing cracked drywall.
The current allergy list had names of children no longer enrolled in the child day center.
Staff #2 was alone with a group of five, toddler children and was not certified in CPR and first aid
The record for staff #2 did not have documentation of education and / or certification required by the job position.
