Inspector Notes

An unannounced inspection was initiated on October 15, 2024 and completed on November 12, 2024 in response to a complaint received by the licensing office on October 1, 2024 relating to allegations of supervision and the center?s policies and procedures. The inspector reviewed two children?s records and one staff record on-site. This inspection included document review, tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to special care provisions and emergencies and are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on November 20, 2024. A POC submitted after this date will not appear on the public website.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence as required by the Standards.

1. The accident report for child #1 did not contain documentation of staff present, any future action to prevent recurrence of the injury, and staff and parent signatures or two staff signatures
2. The accident report for child #2 did not contain documentation of staff present and any future action to prevent recurrence of the injury.
