1. Board Member #2 had an updated screening which was dated 18 days late.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
1. On the roof of building #1, a large piece of the roofing was hanging down the side of the building.
2. The school age playground had broken fence boards and slats missing from the fence.
3. The school age playground had trash which included: plastic cups, chip bags, Styrofoam, and candy wrappers.
