Inspector Notes
An unannounced code compliance visit and facility tour was conducted on 12/13/2022. At the time of inspectors' arrival there were 25 preschool aged children in care with 4 staff members and 3 administrators. Children were observed interacting with staff and engaging in educational activities. A sample of 5 children's records and 8 staff records were reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on record review and interview it was determined the center did not ensure that all staff undergo a background check in accordance with subsection B prior to employment or beginning to serve as a volunteer and every five years thereafter.
Evidence:
1. The records for staff #1 and staff #7 did not contain documentation of updated Central Registry Search results. The documentation available was dated 10/28/16 for both staff members.
