Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Monitor Inspection on April 8, 2024. There were 17 children ages 3 to 6 years old present under the direct care of 5 staff. The Licensing Inspector reviewed 10 children?s records and 12 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Center failed to obtain a copy of the results of a criminal history record information check, a sex offender registry check and a search of the child abuse and neglect registry from any state in which a prospective staff person has resided in the preceding five years.
