Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 06/23/2021 and concluded on 11/01/2021 in response to a self reported incident that occurred on 06/14/2021. A child sustained an injury requiring outside medical attention when he fell off of a piece of outdoor play equipment. Based on the information gathered it was found that the facility failed to maintain sand or other cushioning material under playground equipment.
This violation is listed on the violation notice issued to the facility and was reviewed with administrative staff at the conclusion of the inspection on 11/01/2021.
Violations
1Evidence:
1. During a virtual review of the outdoor play area on 11/01/2021, the inspector reviewed a variety of outdoor play equipment including but not limited to, a set of swings, a little tikes play structure and two small climbing cubes with slides. None of these pieces of play equipment had cushioning material under and around them.
a. A child falling or jumping from any of the outdoor play equipment could sustain an injury.
On Monday, 11/08/2021, landscaping made contact with CCA to inform that the playground will have mulch laid/spread on the grounds on Tuesday, 11/09/2021.
