Inspector Notes
An unannounced complaint inspection was conducted on 08/05/2022 in response to an allegation of inadequate supervision and insufficient resilient surfacing surrounding outdoor play equipment.
Based on the information gathered, which included staff interviews and a review of the program, the allegation regarding inadequate supervision is found to be not valid. Two pieces of outdoor play equipment lacked resilient surfacing therefore, this allegation is found to be valid.
This violation is listed on the violation notice issued to the facility.

Based on observation and staff interviews, the facility failed to maintain sand or other cushioning material under playground equipment.

1. The plastic swing set with attached slide lacked any cushioning material.
2. A small little tikes half cube with attached slide was placed over grass.

Plan of Correction: Administrative staff stated that new mulch was in the process of being spread under all outdoor equipment last Wednesday. It began raining and work was stopped with plans to complete the spread of mulch under the rest of the equipment the week beginning 08/08/2022.
